Planning ahead is one of the greatest gifts we can give ourselves and the people who love us. Join us for this important conversation about essential disability and advance care documents, including Power of Attorney, Durable Power of Attorney, Living Will, and HIPAA Authorization.
For individuals experiencing memory loss, cognitive changes, or a diagnosis such as Alzheimer's disease or another form of dementia, having these documents in place early can help ensure that trusted loved ones are able to support medical, financial, and legal decision-making when needed. Family caregivers and care partners can also gain peace of mind knowing a plan is in place and their loved one’s wishes are clearly documented.
